Medical Case Management

Broadly defined, case management is a range of client-centered services that link clients to resources in the community to help maintain a stable and healthy environment – including healthcare, psycho-social needs and other day-to-day basics.

Clients are asked to participate in completion of a comprehensive assessment of their needs and personal support systems. Afterwards, a comprehensive service plan is developed to guide clients and the case managers in prioritizing needs, identifying barriers to care and linking the client to eligible services. Case managers work with the client to coordinate services. Case managers update client’s service plan on an as-needed basis, but no less than semi-annually.
